Community Health and Health Equity

Our Mission calls us to care for all, especially those who are most vulnerable.

Yet we know it’s not enough to just care for those who are sick. We must move upstream to address the needs of our communities to improve the health of our members and provide equitable access to health care for all.

Health for a better world

A community includes many unique individuals and different groups. Some community members have the resources to afford basic needs, while other members may struggle to put food on the table. In keeping with our Mission to care for those who are most vulnerable, we are working to improve the health of everyone in our community.

Differences in community conditions and experiences among groups within the community have been shown to drive health outcomes. Some populations may experience better, or worse, health based on where they live or the color of their skin. And if a patient doesn’t have access to healthy food or a stable place to live, it is harder to stay well or recover from an illness.

With the help of donations, we work with community partners to help patients in need access social support systems, provide additional support when resources are unavailable, and at times provide one-time support to help them get back on their feet. Your gift can help break down barriers to care that many of our vulnerable patients face.

Funding supportive housing

Housing and health are inextricably linked. Without a safe place to call home, it’s nearly impossible to focus on basic health and medical needs. We are partnering with other agencies to ensure unhoused patients discharged from Providence have a safe, clean environment in which to recuperate with dignity.

Providing patient assistance

The Patient Support Program is used in Providence hospitals in Oregon to ensure that low-income patients have a safe discharge – thus reducing the chance of complications and/or readmissions. The program assists discharged patients with transportation home from the hospital, temporary medications, infant car seats and diabetic supplies. Services are intended to be short term – typically up to 30 days – and function as a bridge to more long-term resources in the community.

Addressing food insecurity

When people are faced with the challenge of buying food or paying for medicine, the need for food often takes priority. This can lead to skipping important medication or delaying health care. Donors fund programs that help our patients secure food and medication, resulting in better health.
